How to Fill-up Inspection Request Template for The First Time

If you are a first-timer in the field of quality you might have questions and unfortunately one of those questions might be this one “How to Fill-up Inspection Request Template?” I felt this when I was a newbie too, later after several months working on our new project and doing the same thing filling up those related templates, months more later, I then became a master of writing descriptions on inspection request, material inspection request etc.

But, there was a case even when I was at the 3 years of work experience, when I joined in a newly opened project I still can’t memorize how to fill the inspection request at the first time. So, it is not a guarantee that when you have the work experience already you can perfectly write it at the first time. That’s why I am writing this article to bookmark on your browser so that once you are about to fill your inspection request you may open this again and then read and follow.

Now, most of the completed works in the construction site shall be inspected by the engineer or consultant. If your project is “Design and Build” as a quality engineer, you will still submit the inspection request to your document controller so that to ensure your internal quality system is moving. And if you have a project direct to the government, in this case, the inspectorate team or concern signatories of such government project shall be the one to check and approve the works.

I’m going to show you now how I fill-up the Inspection Request (IR) form.

1. Filling up the Description of Activity or Work Inspection

The description of activity or work inspection shall be the first to write. This is your goal and intention to have the work approved by the engineer or consultant. Now, remember this that your descriptions of activities should come from the approved Inspection and Test Plan (ITP).

Here is an example of the description of activity, in the ITP, it would be written like “Compaction Test or Field Density Test” but in your inspection request you will write “Inspection for Compaction Test or Density Test Prior to Anti-termite Application.”

2. Writing the Activity and ITP Number

The activity number should be indicated into the Inspection Request and it would determine that where you are in the inspection. And the Inspection and Test Plan number as well, it could signify that the works to be inspected belong to the respective ITP.

3. Location of the work

The third thing that the inspector or an engineer to look at is the location of the completed work that you would want to be inspected. Just make sure that to highlight the area in the drawing and ensure to write the Gridlines of the location and name of the building.

4. The Date of the Submission and the Date and Time of Inspection

The dates and time are really very important to indicate on your Inspection Request. It is stipulated in the Project Quality Plan that the Engineer or Consultant shall be notified 24 hours, it means that you shall ensure that the IR is submitted and that the actual inspection shall be in the next day. The time of the actual inspection must be indicated as it will create conflict to the schedule of the engineer if not orderly scheduled.

The importance of the Inspection Request is immense, as this will serve as the guiding document that a quantity surveyor will consider the release of the filed regular or monthly billings of the stakeholders, subcontractors or the main contractor to the Client. The Quantity Surveyor shall ensure that all the works being indicated in the submitted billings shall be approved via “Approved Inspection Request.
